New Patient Paperwork

Madison Physical Therapy & Consulting, LLC is the only physical therapy provider in Madison to have worked the United State Olympic Committee. We utilize state of the art technology and research supported treatment options to get you better faster!

At Madison Physical Therapy and Consulting, we believe the rehabilitation process is an experiential partnership. We are able to provide more individualized care, more regular appointments and contact with our patients. We are the only facility locally who has invested in the same  equipment as utilized by Team USA to help expedite and you be successful in the healing process.

The initial visit will consist of very thorough evaluation services. A complete physical therapy evaluation should includes the site of the injury and all neighboring joints. For example, with a knee injury, your therapist should also be evaluating your low back, hips, knees, ankles and feet. For a shoulder injury, a thorough evaluation would include your neck, midback, shoulder, elbow, wrist and hand. Without evaluating the entire picture, contributing factors may be missed and delayed results could occur. If you are evaluated for a sports related injury, you can NOT ignore the spine. Nor can you ignore extremities for a spine injury. We look at the complete you and help coach you through addressing any anomalies which are effecting your body as a system.

For our injured workers, a Functional Capacity Evaluation will be based on the job demands. We communicate closely with your care team to determine job specific work tasks for our Functional Capacity Exams. Functional Lift testing may be done during standard physical therapy care and can expedite reporting some job specific demands to your care team.

Upon completion of the initial evaluation, various treatment options will be discussed and outlined. It is important that you are involved in treatment planning based on your therapy goals! A plan of care will be discussed and at that time we can discuss duration of future.

Achieving your physical goals requires investment into the process. Our expectation is that patients will be at the office from 60-120 minutes on average participating in a combination of prescribed strengthening or flexibility to address underlying contributing factors to your injury, hands on care such as soft tissue work or joint mobilization and re-evaluating progress may it be by measurable gains, questionnaires or assessing functional mobility. Use of our state of the art fitness facility is permitted before, during or after your sessions to assist with equipment needs to make progress towards your goals. It is our expectation that you will be working towards cardiovascular health, strength and flexibility as part of the rehabilitation process.
